I Would Like to Serve in Stephen Ministry

Stephen Ministry is composed of men and women trained to provide confidential one-on-one care to people experiencing a difficult time in life, such as grief, divorce, job loss, aging, loneliness, spiritual crisis, chronic or terminal illness, relocation, or separation due to military deployment. Stephen Ministers have a passion for bringing Christ’s love and care to people during a season of need, and a new training schedule is available for those interested in serving.

Including: 
- Studying the material assigned in training 
- Regular visits to your care receiver (weekly or mutually agreed upon frequency) 
- Twice monthly Small Group Peer Supervision and continual education (begins after training is completed)

Due to the nature of the Stephen Ministry, it is necessary to ask the following questions. Please realize that your honesty is very important. An answer of yes to either of the following questions does not in any way rule out the possibility of ministry in this area. A great many caregivers have been made stronger in their care-giving ministry through the care they themselves have received, including mental health professionals. Your Stephen Ministry Team affirms the work of mental health professionals who have helped so many individuals to experience growth and healing. However, if you answer yes to either of the following questions, the Stephen Ministry Team will speak with you about this to better understand its significance in your life and ministry. This application is confidential.

The information I have provided in this application is true and complete to the best of my knowledge. I agree to participate in Stephen Ministry training, in Small Group Peer Supervision, and to function within the boundaries of Stephen Ministry as adopted by my congregation. I give permission for the congregation, if it deems necessary, to call my references, secure a police background check on me, consult with the treating physician(s) and/or other mental health professionals regarding the nature of any treatment I have received for emotional or psychiatric problems, and/or notify the church of which I previously served as a Stephen Minister.
